We have created this privacy statement in order to demonstrate our firm commitment to the privacy of our patrons. The following discloses our information gathering and dissemination practices for www.sph.umich.edu/computing/.

Website Usage Statistics
Our web server logs gather the following information from our users:

Form Data
Our site contains various user request forms which require you to provide contact information (such as name, room number, telephone number, and e-mail address), as well as information on your SPH affiliation (faculty, staff or student and SPH unit). ICS help request forms also ask for information on operating system and computer type. We use this information to:

track and fulfill our service requests
help diagnose problems
bill for services rendered (if applicable)
report data on classroom usage (room requests only)

Surveys
Our annual online surveys ask you for your SPH affiliation (faculty, staff or student and SPH unit). E-mail address is optional. We use survey data to gauge the effectiveness of our service areas.
