Online Surveys

There are several resources available to UM faculty, staff and students who wish to create an online survey:

• UM.SiteMaker sites can be used for online surveys.
• CTools is also available to faculty and staff for creating online tests and quizzes for CTools sites.
• Survey Gizmo is a non-UM tool to create and publish custom surveys that are accessible to persons with disabilities.
• Survey Monkey is a non-UM tool to create and publish custom surveys.
• If you already have an SPH-hosted web site, and would like to have data submitted via an online form sent to an e-mail address, please see our Using Form Mail at SPH page.
• SPH Web Services has developed database-driven web surveys for SPH departments. Please contact us for more information.
