CFPH Application GuidelinesSuccessful candidates to the Certificate in the Foundations of Public Health (CFPH) program have:
*GRE scores are not required; however if you have GRE scores that you would like to submit, you may send them electronically to our Institution Code 1954. Application ProcessOfficial transcripts from all previously attended higher education institutions must be submitted directly to UM SPH. In addition, 2 letters of recommendation (from either professional or academic sources; personal references are not accepted) are required along with a Statement of Purpose (including reasons for pursuing the certificate and career goals). English requirements for international applicants. (Note: Visa requirements do not need to be completed, as the CFPH program requires no travel to campus.) FeesCFPH applicants are required to submit an electronic application. There is a $65 fee due to UM SPH for domestic applicants and $75 for international applicants. That fee is payable online when you submit your application. DeadlinesThe Fall 2012 application deadline is July 15, 2012, and the Winter 2013 application deadline is December 1.
