CFPH Application Guidelines

Applicants to the Certificate in the Foundations of Public Health (CFPH) are required to submit an electronic application via SOPHAS. There is a $95 fee due to SOPHAS, while the UM SPH processing fee is waived for CFPH applicants.

Official transcripts from all previously attending higher education institutions must be submitted to SOPHAS. In addition, 2 letters of recommendation (from either professional or academic sources; personal references are not accepted) are required along with a Statement of Purpose.

This statement should be a concise, well-written essay about your educational background and reasons for pursuing a certificate in public health. It should make the following points. You should give information about your background as it pertains to your academic and professional goals and the motivating influences that stimulated interest in public health. Also, discuss your career goals and objectives and indicate how a credential in public health will help to achieve your educational and career objectives.
Successful candidates for the certificate program have:
• A bachelor’s degree with sufficient preparation in algebra to succeed in the introductory Biostatistics course; sample algebra proficiency exam and answer key (PDF, 7.2MB)
• Attained an overall G.P.A. of 3.0 (B average) in undergraduate study; and
• Proficiency in basic computer skills.
*GRE scores are not required; however if you have GRE scores that you would like to submit, you may send them electronically to our Institution Code 1954.

English requirements for applicants:
Applicants whose native language is not English must demonstrate satisfactory proficiency in English language as measured by the Test of English as a Foreign Language (TOEFL), the International English Language Testing System (IELTS), or the Michigan English Language Assessment Battery (MELAB). Tests must be taken within one year of the term requested for admission.

Applicants should submit TOEFL or IELTS scores via SOPHAS. For TOEFL use Code #5688 SOPHAS, Massachusetts. For IELTS, inform the test center administrator that scores should be released electronically to SOPHAS.

The minimum acceptable TOEFL score required for admission is 560 paper-based, 220 computer-based (CBT) or 100 Internet-based (iBT). The minimum required IELTS score is 6.5 and the minimum required MELAB score is 80.

Applicants who have received degrees from accredited U.S. institutions are not required to submit TOEFL or MELAB scores.

Visa requirements do not need to be completed as the program requires no travel to campus.
