CFPH Application Guidelines

Successful candidates to the Certificate in the Foundations of Public Health (CFPH) program have:

  • A bachelor’s degree with sufficient preparation in algebra to succeed in the introductory Biostatistics course; sample algebra proficiency exam and answer key (PDF, 7.2MB)
  • Attained an overall G.P.A. of 3.0 (B average) in undergraduate study; and
  • Proficiency in basic computer skills.

*GRE scores are not required; however if you have GRE scores that you would like to submit, you may send them electronically to our Institution Code 1954.

Application Process

Official transcripts from all previously attended higher education institutions must be submitted directly to UM SPH. If your transcripts come from outside of the United States, you must submit them for a World Education Services (WES) course-by-course evaluation.

In addition, 2 admission recommendation forms (from either professional or academic sources; personal references are not accepted) are required along with a Statement of Purpose (including reasons for pursuing the certificate and career goals). Letters of recommendation from the 2 recommenders are optional and encouraged.

English requirements for international applicants. (Note: Visa requirements do not need to be completed, as the CFPH program requires no travel to campus.)

Fees

CFPH applicants are required to submit an electronic application. There is a $65 fee due to UM SPH for domestic applicants and $75 for international applicants. That fee is payable online when you submit your application.

Deadlines

Our Fall 2013 deadline is July 15th, 2013. We will begin to review completed applications in mid-May.

Our Winter 2014 application will not be available until July.
