The application form must
be completed in its entirety. It must be indicated on the application
whether or not courses are to be taken for graduate credit. If
graduate credit is requested, a photocopy of proof of last degree
conferred must be included with the application. Failure to
do this will delay the review and acceptance of the application.
The application form must be accompanied by a $50 application
fee (check payable to the University of Michigan in U.S. funds),
which is non-refundable. A late fee of $50 will be applied for
any applications postmarked after June 1, 2008. MAILING
INSTRUCTIONS ARE FOUND ON THE APPLICATION FORM. Before completing
the application, please note carefully the schedule of courses
and avoid selecting courses that are offered simultaneously.
Participants financed by such
organizations as the federal government, WHO, etc., should arrange
with their agencies to provide supporting documents with submission
of their applications. All registrants should be prepared to
provide payment to the University of Michigan for the balance of
their tuition in U.S. dollars by June 15, 2008. Checks, cash,
money orders, Visa and Mastercard will be accepted. Payment of the balance of fees
in advance will expedite the registration procedures. PURCHASE ORDERS ARE NOT ACCEPTED FOR TUITION.
Payment in Full
Full payment for tuition must be postmarked
on or before June 15, 2008. Any payments after June 15th will
be subject to a $50 fee.
Application
Deadline
Enrollment is limited in a number of
courses. Applications must be received by June 1, 2008. A late fee
of $50 will be applied for any applications postmarked after June
1, 2008. Applications received by the deadline does not guarantee
placement in a course. If a course becomes full before the application
is received, applicants will be placed on a waiting list and notified
when space becomes available.
Continuing Medical Education Accreditation
This activity has been planned and implemented in accordance with the Essential Areas and policies of the Michigan State Medical Society Committee on CME Accreditation through the joint sponsorship of the Public Health Consortium and the Graduate Summer Session in Epidemiology. The Public Health Consortium is accredited by the Michigan State Medical Society Committee on CME Accreditation to provide continuing medical education for physicians. The Public Health Consortium designates this activity for a maximum of 105 AMA PRA Category I credit(s)TM. Physicians should only claim credit commensurate with the extent of their participation in the activity.
The Public Health Consortium designates this education activity
for a maximum of 105 credit hours out of a total of 655 presented
in Category I toward the AMA Physician’s Recognition Award.
Each physician should claim only those hours of credit that he/she
actually spent in the educational activity. CME credit is issued to physicians only. A fee of $100 per person will be assessed to cover the administrative costs related to CME credit.
Computer Connectivity (for non-credit participants only)
Participants who want to use University of Michigan computing facilities will need to purchase a Basic Access Computing Package for a cost of $15.50. Basic access to computing services means you can use a Campus Computing Site or University Library machine to access the web and your own (non-UM) e-mail account. You will be provided an I.D. number and password for the duration of your attendance of the program. Please be sure to indicate on the applicaiton that you are requesting this service.
Final Registration
Final registration will be held from 1:00 p.m. until 4:30 p.m. on Sunday,
July 6, 2008, at the Crossroads, on Washington Heights, adjacent to the Medical Campus and University
of Michigan Hospitals. This will be followed by a reception. All
faculty and registrants are urged to make their travel plans so
that they arrive in time to receive course materials and new information
about the program. Registrants whose first day of class begins after
July 7th will register on the first scheduled day of class between
7:00-1:00 p.m.
Cancellation and Refund Policy
Cancellations must be received in the Graduate Summer Session Administrative
office in writing and postmarked on or before July 1, 2008. Cancellation
requests should be addressed to Jody Gray and must include a reason
for the cancellation. The $50 application fee is non-refundable.
Refunds will be processed within 30 days following the end of the
program (July 25, 2008). The Graduate Summer Session reserves the
right to cancel any course due unforeseen circumstances.
Graduate
Course Credit Information
Courses have been evaluated with respect to their equivalency to
the University of Michigan campus-based day classes. The requirements
of course content, homework, participation in class, and grading
are reviewed by the Curriculum Committee granting credit. Acceptance
of courses for credit or substitution for similar courses offered
by an institution is dependent upon the policy of that institution.
Certificate of Participation
Participants registering for the Graduate Summer
Session in Epidemiology may register for credit or non-credit. A
certificate of participation will be issued to those registering
for non-credit, as well as to those registering for credit. Non-credit
participants are expected to complete all course exercises but are not required to take examinations. Course credit hours assigned
by the University of Michigan are indicated in the course descriptions.
Letter grades will be issued for all courses. Official grade reports
will not be available until early September. Grades will not be issued for non-credit participants.
