Form WizardThe Form Wizard will appear. The first step is to select which fields you want to include in your form. Select single fields by highlighting the field you want in the "Available Fields" list (1), and then click the right arrow button (2) to move it to the "Selected Fields" list. You can select all the fields from your table by clicking the double arrow button (3). When you are finished, click the Next button (4).
