Instructions:

There are 15 survey sections covering different types of beautification activities (see the list below).

You will complete all 15 survey sections, even if you have beautification activities related to only some of the survey sections. For each survey section, you will answer a screening question indicating if your project has conducted that type of activity.  If you answer “no,” you will be skip to the end of the survey.  If you answer “yes,” you will answer all the questions for that section.

Before you begin each survey section, download and complete the survey section worksheet. Worksheets are downloadable WORD files. Click the WORKSHEET button to download and print the worksheet.  You should complete the worksheet first so you can gather all the required information for that survey section. 

A few grantees will need to complete and upload one or more EXCEL spreadsheets for some survey section questions.  This requirement only applies to the grantees who report a very large number of grant activities for a specific survey question.  Specific instructions in the survey section worksheets will help you determine if this requirement applies to your grant.  The downloadable EXCELl Spreadsheets with required data fields can be accessed by clicking the SPREADSHEET buttons below.

After you complete the survey section worksheet, click the GO TO SURVEY button to begin entering your information into the online survey section. 

IMPORTANT TO KNOW:

• Once you start a survey section, you must finish it.  If you exit the survey section before submitting the survey, you will lose all your work on that survey. You do not, however, need to complete all 15 survey sections at the same time.


• You will need to use a computer with a printer for completing your surveys.  Once you have answered all the questions in a survey section, you will view a Summary Page with all your responses that you should print for your records.
