Fall 2008 Update

As of 5/1/08: Fall 2008 applications are no longer being accepted for the following programs:

  • Epidemiology: General Epidemiology M.P.H.
  • Epidemiology: International Health M.P.H.
  • Epidemiology Ph.D.
  • Health Behavior and Health Education M.P.H.
  • Health Behavior and Health Education Ph.D.
  • Health Management and Policy Ph.D.

All other programs and degrees are still accepting applications for Fall 2008.

We encourage you to complete your application online. Using the websites below, you can work on and complete your application from any location with Internet connectivity. Your answers are saved and available to proofread until you are ready to submit your application. The application fee is paid by credit card via a secured site. With your online application, you will need to submit transcripts and supplemental materials in hard-copy format.

Deadlines for applying to an M.S. or Ph.D. program vary and are available at the Rackham School of Graduate Studies page of deadlines.
Apply online to the Certificate in the Foundations of Public Health

Applications for Fall 2008 are still being accepted.

Printable M.P.H. and M.H.S.A. application forms are available, and printable M.S. and Ph.D. application forms are available at the Rackham School of Graduate Studies web site.

You may submit hard copy materials for M.P.H., M.H.S.A., or Certificate in the Foundations of Public Health (CFPH) applications to: Admissions Office, University of Michigan School of Public Health, 109 Observatory, Ann Arbor, MI 48109-2029.

M.S. and Ph.D. hard copy materials should be sent to the Rackham Graduate School and directly to the department. M.S. and Ph.D. application materials should not be sent to the UM SPH Admissions Office.
